This packet is provided in response to your organization’s request to be considered for an award of funds.  We are confident that the programs or services your organization proposes and supports are in keeping with our stated mission:

“To improve the lives of persons 60 years of age or older by aiding with health, nutrition, transportation and other quality of life issues”

Make sure that you thoroughly read and understand the Guidelines and General Assurances. The deadline for receiving requests for funding is January 8, 2024.  Your completed request should be mailed to the address in the above letterhead.

When the Board of Directors has received your organization’s completed forms, we will review them along with any and all other requests that are submitted.  Following that review we will notify you if your request will move to Phase 2 of our approval process.  Phase 2 will include an interview with a senior management individual from your organization who has the authority to make financial policy decisions.  You should plan on a presentation/question and answer session that will last from15-30 minutes.  We will contact you to schedule a time for your interview.

By the end of January 2024, we will begin notifying organizations that are successful in receiving an award of funds.  Please remember that we have limited funds available to award.
